Criteria For 2010/2011 Plan Year
  • Complete at least one of the following Wellness Programs (enroll before Jan 4, 2010):
  • EMPOWERED Coaching (13 Focus Areas)    Completion criteria: have at least three contacts with your health coach before March 31, 2010.
  • Walk this Way
    Completion criteria: pursue 360,000 steps by March 31, 2010.
  • Condition Management Coaching (7 Condition Areas) Completion criteria: complete at least three coaching calls before March 31, 2010.
