Subject: ACH forms

Please direct this message to the correct person(s) in your agency who is responsible for obtaining ACH forms (with applicable banking information) from vendors and other payees.

Please see the attached ACH form that has been approved by State Accounting and the State Treasurer’s office.  Agencies should not be currently using older versions of this form.  If you have older versions on hand, please destroy them and begin using the current version.  The current version is available on both the State Accounting and the State Treasurer websites.

If you agency has made changes to the prescribed version for any reason, please email a copy of your modified form you are using to tmstaff@treasurer.org.
